I will not come to my appoinment if I am feeling ill. I will advise my artist as soon as possible if i need to reschedule. 

I agree to come to my appointment alone unless all people in my group are getting tattooed that day or I have advised my artist I will be bringing someone to accompany me. 

I agree to sanitize my hands with provided hand sanitizer upon arrival.

I agree to eat at least 3 hours prior before my appointment (this helps your body deal with endorphins produced during proceedure)

I understand that not all tattoos are going to take exact amount of time quoted. Unexpected time with stenciling or freehanding may add time to my appointment. I ensure i will have enough time available & understand this process will not be rushed. I will try not come too ealry or late but as on time as possible. 

I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about obtaining a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I have been advised of the facts & matters set forth below and agree as follows:

If I have diabetes, hepatitis, hemophilia, HIV-AIDS, or any other communicable disease, heart condition or I take medicine which thins the blood I have advised my tattoo artist.

I am not under the influence of alcohol or drugs

I am not pregnant or nursing

I acknowledge it is not reasonably possible for Royal Anchor Tattoo Inc. or contractors (your tattoo artist) To determine wether I might have an allergic reaction to the pigments or process used in my tattoo. I agree to accept the risk that such a reaction is possible.

I acknowledge that infection is always possible as the result of obtaining a tattoo, particularly in the event that i do not take proper care of my tattoo. I have received after care instructions and I agree to follow them as my tattoo heals. I agree that any touch-up work needed as the result of my negligence or failure to follow such instructions, will be done at my own expense.

I acknowledge that a tattoo is a PERMANENT change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo. To my knowledge, I do not have mental or medical impairment or disability which might affect my well-being as a direct or indirect result of my decision to have a tattoo. By going ahead with my tattoo after discussion with my tattoo artist, I approve the placement, design, SPELLING, & colours used in the tattoo.

I agree to have Royal Anchor Tattoo Inc. and all it’s contractors (tattoo artists) to use my likeness and pictures of my tattoo for any publication. By entering Royal Anchor Tattoo i realize security video cameras will capture my likeness and stream content offsite.

I acknowledge I am 18 years old and of age, and consent to obtain a tattoo. I have truthfully represented my age to Royal Anchor Tattoo Inc or the tattoo artist applying the tattoo. I agree that obtaining this tattoo is my choice alone. I consent to the application of the tattoo and to any actions or conduct of the representatives and contractors of Royal Anchor Tattoo Inc reasonably necessary to perform the tattoo procedure.

Therefore, I request the tattoo artist checked above to execute my tattoo. I understand this type of tattoo usually takes an average of 2 weeks to heal. I hereby release and forever discharge and hold harmless Royal Anchor Tattoo Inc, the tattooist and all affiliate, owners, managers, and contractors from any and all claims, damages or legal actions arising from or connected in any way to my tattoo, to the fullest allowed By the law.

I agree my deposit be forfeit if I cancel/no show without 48hrs notice, if I arrive sick, if I do not follow the precautions set by Royal Anchor Tattoo Inc. I understand deposits are the responsibility of the contractor (tattoo artist) and Royal Anchor Tattoo Inc has no dealings with deposits agreed upon with your artist.

By signing this agreement, i agree that I have read the Tattoo Waiver provided By Royal Anchor Tattoo Inc and agree to its terms.

I agree to adhere to all social distancing protocols whenever possible. I agree to hygiene protocols; to arrive showered and with clean clothes. (Please do not come directly from work)
