PARTICIPANT AGREEMENT, RELEASE AND ASSUMPTION OF RISK

THIS AGREEMENT AFFECTS YOUR RIGHTS. READ CAREFULLY BEFORE SIGNING.

I/You agree to the terms of this Agreement for the Deviant Ball Fetish Event occurring on December 19, 2025 at Walter Studios, 747 W Roosevelt St, Phoenix, Arizona 85007 (the “Event”), operated by Walter Productions LLC and Black Dragonfly Studios LLC (collectively the "Organizer"), in consideration of being allowed entry to provide services for, and/or attend the Event and hereby covenant and agree as follows:

1. Eligibility and Voluntary Participation

I affirm I am at least 21 years of age. I understand that attendance at the Event is completely voluntary and that I may leave or decline to engage in any activity at any time. I acknowledge that the Event may involve adult-oriented themes, including consensual kink or fetish-related activities, and I agree to participate only to the extent they feel comfortable and safe.

2. Personal Responsibility, Boundaries, Rules, and Conduct

I agree to engage in any activity only on a fully informed, freely given, and revocable consent basis. I agree to respect the boundaries, limitations, and consent of all others at the Event. I accept full responsibility for monitoring my own physical and emotional condition and agree to stop any activity that feels unsafe or non-consensual. I hereby acknowledge I have read and received a copy of the Event rules and agree to comply with all Event rules, safety protocols, consent policies, and instructions provided by the Organizer or Event staff. I agree to respect the privacy and anonymity of all attendees. I will not record, photograph, or otherwise document any person or activity without explicit prior consent and compliance with Event rules. I UNDERSTAND AND AGREE THERE IS NO SEXUAL INTERCOURSE, PENETRATION, OR GENETIAL EXPOSURE ALLOWED. I understand the Organizer reserves the right to remove me or any participant who violates Event rules or engages in unsafe, coercive, disruptive, or harmful behavior, without a refund.

3. Assumption of Risk

I understand and acknowledge that my participation in the Event involves inherent risks, dangers, and hazards, including but not limited to consensual sexual activity, erotic interaction, power exchange dynamics, and the use of fetish-related implements or equipment. Participant voluntarily assumes all risks associated with such activities, which may include, but are not limited to:

• Physical injury, including minor injuries, bruising, abrasions, muscle strain, nerve irritation, or other unintended bodily harm, serious injuries, and even death, resulting from activities, interactions with other participants, equipment, or the venue itself
• Risks associated with restraint, impact play, sensory play electrostimulation, tattoo procedures, or other physical interactions
• Risks arising from the use or misuse of fetish tools, props, furniture, or devices, whether provided by the Organizer or other participants
• Emotional distress, discomfort, embarrassment, psychological harm, sensory stimuli, or discomfort
• Exposure to sexually explicit or suggestive content, activities, and environments
• Exposure to bodily fluids, intimate contact with others, and the risks of sexually transmitted infections, and other infectious diseases, despite any precautions taken
• Risks associated with the consumption of alcohol or other substances, if permitted at the Event
• Loss or damage to my personal property, and
• Risks associated with the actions or inactions of other participants, staff, or third parties.

I voluntarily accept and assume all risks, both known and unknown, associated with sexual play, fetish tool use, and my participation in the Event, even if arising from the negligence of the Organizer or others, and assume full responsibility for my participation. I understand the Organizer makes no guarantees or warranties regarding my safety during the Event, does not supervise, instruct, or control private interactions between participants, is not responsible for the actions, omissions, or skill level of other participants, and I acknowledge my attendance at my own risk.

4. Release and Waiver of Liability

In consideration of being permitted to participate in the Event, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue the Organizer, its officers, directors, employees, agents, volunteers, and affiliates (collectively, the "Releasees") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the Releasees or otherwise, while participating in, traveling to, or traveling from the Event, or while in, on or upon the premises where the Event is being conducted.

5. Indemnification

I agree to indemnify, defend, hold harmless, and promise not to sue the Releasees from and against any and all claims, demands, liabilities, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or in connection with my participation in the Event, including but not limited to, any claims brought by third parties as a result of my actions or inactions.

6. Medical Disclaimer

I affirm that I am in good health and physical condition and do not have any medical conditions, including contagious and/or infectious diseases, that would prevent me from safely participating in the Event. I understand that the Organizer is not responsible for providing medical care but will make reasonable efforts to provide basic first aid and life saving support as reasonably possible.

7. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Arizona, without regard to its conflict of laws principles.

8. Entire Agreement

This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, with respect to such subject matter.

9. Severability

If any term or provision of this Agreement is held to be invalid, illegal, or unenforceable, the validity, legality, and enforceability of the remaining provisions shall not in any way be affected or impaired thereby.

10. Acknowledgment

I acknowledge that I have read this Agreement, understand its terms, and am signing it freely and voluntarily below. I further understand that by signing this Agreement, I am giving up substantial legal rights.
